provides health support services at no additional cost to participants:

Your own Personal Nurse
24-hour / 7 days-a-week nurse phone line support
Nutrition and lifestyle information
Home visits, should the need arise
Group classes on health topics

Medicare Health Support in west central and southwest Florida is a new health improvement pilot program offered to invited fee-for-service Medicare beneficiaries with congestive heart failure and/or diabetes, among their conditions.

Medicare Health Support is designed to help reduce health risks, improve quality of life, and promote optimal health.

Medical Health Support does NOT replace any existing coverage you now have, and there is no additional cost to participants. This program is voluntary and provides participants with added health education and support services.

Note, the resources and services described on this web site are available only to people who have been invited by Medicare to participate in this Medicare Health Support program.
